TATTOO CONSENT FORM

I understand that with FINGER/HAND/FOOT tattoos that there is a high chance the pigment will not stay it is likely to wear significantly and if I want it touched up this will be at my own cost this is not at the fault of the artist.

I understand that if I am on any medication for e.g. diabetes/anti depressant tablets and a wide range of other medications it is  likely my tattoo will have  poor, delayed healing, extra scabbing & a higher risk of infection.

I understand the risks of a tattoo are the following: allergic reactions/skin infections/swelling/burning/keloid scarring/discomfort or pain during the tattoo process/the need for touch ups additional sessions

I understand that absolutely everything my artist uses is completely sterile or disposable leaving no risk for infection during the procedure and that it is my responsibility to keep my tattoo clean once I leave the studio, it is an open wound. (In the very rare case you do end up with an infection please do not hesitate to contact the studio who will know exactly how to advise you there will also be information in your aftercare instructions - do not panic) if you have any worries at all contact us at any time we will help you.

I agree to S-line studios taking a photo of my tattoo and posting it in their website/socials

I am not pregnant/breastfeeding 

I confirm I am over the age of 18 and consent to having this procedure

I confirm I have not taken drugs or alcohol in the last 24hrs and understand that doing so after the tattoo will affect the healing as will smoking.
