By checking, I agree to:

1. Voluntary Procedure

I understand that getting a piercing is voluntary, and I have not been coerced into this decision.

2. Risks & Complications

I acknowledge that body piercings carry inherent risks, including but not limited to:

Infection

Allergic reactions

Swelling or prolonged healing

Scarring or keloids

Nerve damage or migration/rejection of jewelry

I accept full responsibility for any complications that may arise.

3. No Guarantees

I understand that individual healing times and outcomes vary. The piercer and studio do not guarantee the final result or the healing process.

4. Aftercare Responsibility

I have been given aftercare instructions and understand that proper care is my responsibility. Failure to follow instructions may result in complications.

5. No Refunds

I understand that no refunds will be given for services performed. Jewelry purchases are final sale due to hygiene reasons.

6. Parental Consent (For Minors)

(If applicable) As the parent/legal guardian, I give full permission for them to receive a piercing. I understand and accept all risks.

Legal Release & Waiver of Liability

I release S-Line Studios, its employees, and piercers from all liability for any injuries, infections, reactions, or complications arising from my piercing. I agree not to sue or hold the studio responsible for any damages.

I confirm that:

✔ I am at least 18 years old (or have parental consent).

✔ I have disclosed all relevant medical history.

✔ I have been fully informed of the risks and responsibilities.

✔ I am not under the influence of drugs or alcohol.

✔ I understand that piercings are permanent and may leave scars.
